Message from the Board

iCircle Services has grown to 85 Providers in its network! We are proud to have such an extensive and diverse group of agencies working with us. As a reminder from our recent Provider Meetings, if you know of any agencies that would be interested in joining our MLTC Provider Network, please put them in touch with us.

Also, we are looking forward to the focus groups that are beginning next week. This will be a great opportunity for us to get to know some of the individuals and families who may be served by iCircle in the future, and to gain additional insight from them on how the DISCO can best support them as they transition to the new system.

We have been taking some time to review the feedback from the Provider Survey and we look forward to sharing the results with you in next week's issue of Provider Network News.

iCircle Services is proud to be a sponsor of the 2013 Statewide Conference of the New York Association on Independent Living (NYAIL). NYAIL's 2013 Statewide Conference will be held September 10-11, 2013 at The Hilton Garden Inn & The Rensselaer Banquet and Conference Facility in Troy, New York.

About the Conference
More than 250 people with disabilities and older New Yorkers will be celebrating 35 years of Independent Living in New York at NYAIL's 2013 Statewide Conference. Thirty-five years ago, the first Independent Living Center in New York was founded in New York City. Today, over 30 ILCs across the state continue to champion the rights and equality of all people with disabilities with a younger generation of leadership emerging. While the concept of Independent Living has expanded and become more widely recognized, there remains much work to be done to continue ensuring equal opportunities for all people with disabilities, as well as promoting the Independent Living philosophy both within the disability community and to the public.

Who Should Attend
The conference will provide valuable information to people with disabilities and their families, board members and staff of Independent Living Centers and other disability rights organizations, vocational rehabilitation professionals, policy makers, older New Yorkers, youth with disabilities, government agency representatives, and local business professionals.

Provider Spotlight: Hurlbut Care Communities

Established: 1969, under the name ROHM Services
 
Mission: At Hurlbut™ Care Communities, our family of compassionate professionals provides person-centered care to our residents and support to their families – to enrich body, heart, mind and spirit.
        
Executive Director/CEO: Robert W. Hurlbut
 
Developmental Disability Services: Short term rehabilitative services and long-term care for persons of all degrees of mental capacity.  Hurlbut’s team develops customized treatment plans, and works closely with the patient’s physician, specialists, family, and other providers to establish attainable goals and individualized care plans. They offer Physical Therapy, Occupational Therapy, Speech and Language Therapy, and Neurological Rehabilitation.
 
Other Services: Diabetes Specialty program at The Hurlbut, as well as the ACP Program (Accelerated Care Plus) which is a specialized therapy program. All facilities also offer Physician and Social Work services, Dietician services, Audiology, Dental, Podiatry and mental health services.  
 
Number of Employees: 1,327 employees
 
Number of Individuals Served: With over 750 total beds, over a thousand people are served each year
 
Coverage Area: Facilities are located in Monroe County, Livingston County, Seneca County, Steuben County, and Wayne County (as well as Palm Beach County, Florida)
 
Ages Served: Adults ages 21 and up

Provider Spotlight: Toomey Residential & Community Services

Established: 1990; an affiliate of Catholic Charities of Onondaga County

Mission: To help individuals achieve their highest level of capability and increase opportunities for their future success. Toomey is committed to person centered, family focused services.

Executive Director: Judith D’Amore

Developmental Disability Services: Two intermediate care facilities; four individual residential alternatives, day habilitation without walls, community habilitation and Medicaid Service Coordination

Other Services: Foster care, both general and therapeutic in foster boarding homes; Unaccompanied Refugee Minor therapeutic foster care, four agency boarding homes for teens and a supervised independent living program for transition age teens ages 18-21; Office of Mental Health services consist of a community residence for boys ages 7-13 and OMH HCBS Waiver respite and skill building; the nursing department provides medical case management and staff training for all of the individuals in Toomey’s care.

Number of Employees: 160

Number of Individuals Served: 100

Coverage Area: Onondaga, Oneida, Madison, Cayuga, Broome, Oswego, and Cortland counties

Ages Served: Within their scope of services, Toomey serves all age groups, infants through adulthood.
