Message from the Board

As iCircle representatives have traveled around the network, many questions have come up about the options available for people to have more input and control over their services. In addition, there were many questions about the Front Door that has been established at OPWDD. As more individuals, families and providers join the iCircle newsletter list, we thought that it would be helpful to provide some quick links to a the presentations on Self-Direction and the Front Door developed by the OPWDD:

We encourage providers to continue to communicate with individuals and families during this time of transformation.

Seeking MLTC Service Providers

iCircle is working with the Department of Health on its Managed Long Term Care (MLTC) application. Once we have been approved, we will be able to provide comprehensive care management services for Medicaid eligible individuals who are 21 years and old and require more than 120 days of community-based home care services, as defined by the NYS Department of Health. The MLTC plan will include services like:

  • home care
  • personal care
  • adult health
  • social day care
  • nursing home care
  • specialty health
  • other services (i.e. home delivered meals, transportation, and personal emergency response)

We are currently working to build our MLTC provider network and it is our hope you can help! We are interested in hearing from you about the providers you trust, that serve your area and provide you or your family members with any of the services listed above. Please email info@icirclecny.org with any feedback or to tell us more about providers in your area.

For those providers already in our network who offer any of the services above, please be aware that you will be contacted by Independent Living Systems (ILS), iCircle's managed care partner, as part of the effort to establish our MLTC Provider Network. CQL Personal Outcome Measures® Interview Assessment Workshops

A four-day workshop that covers how to conduct the Personal Outcome Interview, synthesize interview data from people served, analyze findings, and identify organizational needs for change.

Who Should Attend: Quality Assurance Specialists, Medicaid Service Coordinators, Managers, Clinical Staff & Direct Support Professionals

Why: OPWDD will be utilizing Personal Outcome Measures® to evaluate agencies quality of services.

What: At the completion of this four- day workshop, you will be able to conduct Personal Outcome Measures® Interviews and learn how to utilize the data for individual plan development and organizational trending for enhancement of quality services.

  • Interim HealthCare is a NYS DOH approved provider for the Traumatic Brain Injury (TBI) Medicaid Waiver Program as well as the Nursing Home Transition and Diversion (NHTD) Medicaid Waiver Program. They also participate in the Medicaid CDPAP program.
  • Provide a broad array of skilled home care and non-medical supportive services, including but not limited to: Hi Tech Nursing (I.V. infusion), Ventilator, Ostomy teaching and management , Wound care , Nursing assessments and monitoring, Medication administration and treatments, and Lab draws

Coverage Area: Rochester Area Counties - Monroe, Livingston, Ontario, Genesee, Steuben and Wayne; Downstate Counties - Orange, Rockland, Sullivan, Dutchess, Ulster, Putnam, Westchester, Bronx, Queens, Nassau and Suffolk

Ages Served: Newborn through geriatric age groups
